Appointment Reminders & Cancellation Policy
We will call you 24 hours before your scheduled appointment. If you cannot make your scheduled appointment and need to reschedule, please call us with 24 hour notice.
There is a $75 charge for not giving a minimum of 24 hours notice for cancellation of an appointment or no-show for an appointment. No new appointments will be scheduled until this is paid.

We Want Time to Hear Your Concerns
In order to allow you the fullest amount of time to converse with the audiologist about your hearing concerns, it is recommended that all new patients complete the forms provided in this section prior to their initial visit. You may also call our office if you would like these forms mailed to you.
Tending to these forms in advance will also allow you more time to gather the information requested and seek help from a family member if necessary.
All forms can be returned by regular mail at the address provided at the top of this page, by emailing them to info@audiologyspecialists.com, or dropped off anytime at our office.

HIPAA Policy
2. Many insurances require a referral, or Doctor’s Note, for them to cover a hearing evaluation. It is necessary for patients to contact their provider and request this paperwork be sent to us. It is the patient’s responsibility to make sure this paperwork was properly sent and is dated before the appointment. We can confirm if we have received it but we can’t request the paperwork. Some insurances require a physician note of “Medical Necessity” before covering the purchase of hearing aids. This note is also the patient’s responsibility to obtain.
3. If paperwork doesn’t arrive with the correct date, ie, if it is dated after the day the evaluation was performed, patients will need to pay for their appointment out of their own pocket.
4. We will advise you, to the best of our knowledge, what benefits you may or may not have. However, patients are responsible when insurance companies mis-advise us or change their mind. In the event that a service or product is not covered, patients will be responsible for the full expense. Insurances have the right to change their mind for 18 months after the service. This occasionally results in a reversal of payment. If necessary, we can advise you how to set up a Care Credit account to help you manage such an expense.
5. We must charge everyone the same amount, or it is called fraud. However, we offer discounts for payments at the time of service. For example, a hearing evaluation is billed at a set amount. However, if paid at the time of service, and not billed to insurance, there is a discounted price. This is a reward for prompt payment.
6. Patients are responsible for charges denied by their insurance coverage.
7. We accept all major credit cards, including Care Credit. There is a 3% surcharge fee added to all credit card and Care Credit transactions. There is NO surcharge fee for payments made by debit card, cash or check.
8. We will never sell your information
9. We will always do our best to help you and improve your hearing in any way we can.
10. Disrespecting or yelling at any member of the Audiology Specialists staff will result in termination from our practice.
