We are in the office for Administrative purposes on Wednesday and Friday 9am-2pm

Late hours on Tuesday and other appointments, outside of regular patient hours, may be scheduled by prior arrangement.

New Patients

We Want Time to Hear Your Concerns

In order to allow you the fullest amount of time to converse with the audiologist about your hearing concerns, it is recommended that all new patients complete the forms provided in this section prior to their initial visit. You may also call our office if you would like these forms mailed to you.

Tending to these forms in advance will also allow you more time to gather the information requested and seek help from a family member if necessary.

All forms can be dropped off at our office, brought with you to your appointment or they can be sent to us via postal mail or via email. Please remember that email is not HIPAA compliant and some messages are lost to the Ethernet.

Appointment Reminders & Cancellation Policy

You will receive a text message and an email, if provided, 2 days prior to your appointment as a reminder. If you cannot make your scheduled appointment and need to reschedule, please call us with 24 hour notice.

There is a $75 charge for not giving a minimum of 24 hours notice for cancellation of an appointment or no-show for an appointment. No new appointments will be scheduled until this is paid.
