Patient Forms

We Want Time to Hear Your Concerns

In order to allow you the fullest amount of time to converse with the audiologist about your hearing concerns, it is recommended that all new patients complete the forms provided in this section prior to their initial visit. You may also call our office if you would like these forms mailed to you.

Tending to these forms in advance will also allow you more time to gather the information requested and seek help from a family member if necessary.

All forms can be returned by regular mail at the address provided at the top of this page, by emailing them to info@audiologyspecialists.com, or dropped off anytime at our office.

TESTIMONIALS

Wonderful Visit!

“I previously had a very unpleasant experience in a hot, claustrophobic room at another ear doctor’s office. I was so much happier with my experience at Audiology Specialists; I couldn’t have been more comfortable. Dr. Robertson was wonderful!”

Impressed With the Care!

“I have been impressed with the care and service I receive from Dr. Robertson at Audiology Specialists, LLC. I was recently told by my friends that it was very nice to no longer have to repeat themselves to me.

I am really pleased with my hearing aids. They work so much better than the ones I had purchased elsewhere, a couple of years before. With these instruments, I was able to understand the announcements and pages made over the PA system at Foxwoods.””

-Bruno S., Moultonboro, NH

Compassionate Care

“The gentleness and respect that was shown by the staff at Audiology Sepcialists, so gentle, so compassionate, so like I would hope my own mother would be treated, I came away impressed and feeling so good about our local businesses.

Oh, by the way, I too left the business totally satisfied. Hearing issues? This is the business for you.”
